ADULT & CAREGIVER SURVEY
The Iowa County Healthy Aging in Rural Towns “HeART” Coalition invites you to participate in a survey that will be used to design projects that will increase support for older adults and caregivers in Iowa County. If you consider yourself an older adult and reside in Iowa County you are eligible to complete the Adult Survey.
Many of you fill the role of caregiver if you are helping an older adult stay in their home by taking them to appointments, providing meals, managing their medications, doing household tasks, etc. If you are a caregiver (outside of your employment) please complete the Caregiver Survey.
Some of you may be eligible to complete both surveys.
To participate in the survey go to https://care.nursing.wisc.edu/heart/. Scroll to the bottom of the web page to open either the adult or caregiver survey.
Once you have completed the survey, call or email Kathy Key at the Health Department to have your name entered into a drawing for (2) $100 prizes.
Kathy.firstname.lastname@example.org or (608) 930-9870
Please feel free to forward this email on to other older adults and caregivers that you know living in Iowa County. We would love to hear from as many people as possible.
